If you are injured at work or while performing work duties away from work, you must notify your employer immediately. Your injury may have occurred due to sudden trauma or due to the accumulation of smaller injuries over time. As soon as you are aware that something is not right, tell your employer. Your employer is required to file a claim with their insurance carrier and provide you with a copy. If they have not done this, you should contact the Vermont Department of Labor and request a Form 5 – Employee’s Notice of Injury and Claim for Compensation.
You should get medical care as soon as possible. If you are unable to work, you will need a note from your doctor documenting that. You may be directed to a company doctor for the first visit. Following that visit, you can choose a doctor. You need to file a Form 8 to change your doctor. You can get the form from the insurance company or if you need to from the Department of Labor.
Your company's workers' compensation insurance company has 21 days to decide if your injuring is work-related. They will send you a Form 7 Medical Authorization to allow them to access your medical records relating to your injury. You should keep them informed about any medical appointments you have. You have the option to choose chiropractic care if your injury warrants it.
